Fremont Cares: Mark's New Beginning at Natalie's Place

For many years, “Mark” lived at the Ardenwood encampment. 

During this time, a dedicated team of partners supported his journey toward stable housing. A Human Services case manager coordinated services and helped Mark stay connected to the resources he needed, and a medical social worker from Bay Area Community Health met with him weekly to address his ongoing health concerns. Additionally, an Abode outreach worker provided consistent regular contact, keeping Mark informed with important updates and connecting with his care team as he prepared for his next step.


When news arrived that Natalie’s Place would begin accepting residents, the team came together quickly to help Mark secure a placement. 

Cardea Health, the nonprofit operator of Natalie’s Place, met with Mark and welcomed him in for a tour of the facility. Staff explained the housing and supportive services available to residents, including a private room and full bathroom, on-site laundry, medical transportation, ADA accommodations, clinical staff to coordinate care plans, three meals per day, 24/7 on-site safety ambassadors, and program management to support resident needs and maintenance.

Initially hesitant about the move, Mark asked questions, considered pros and cons, and advocated for himself. With support from his Human Services case manager, he ultimately committed to making the move. While the strong support of Mark’s care team was essential, his own persistence ultimately made the difference. 

Thanks to the team’s collaboration and Mark’s determination, he became the first resident to move into Natalie’s Place, marking an important new beginning in his journey to permanent housing. 

The team’s support continued after Mark’s move, with staff maintaining his information in the coordinated entry system as well as updating his documents needed for permanent housing. 

In late August, Mark matched with an apartment, and because his information and documents were current and ready, he was able to move into permanent housing right away.

Mark’s story reflects the power of continued collaboration, steady support, and most importantly, his own determination to take the next step toward long‑term stability.
